Summary:

This pharmacist will be part of a dedicated team withinthe Emergency Department as part of a fast-paced, collaborative,multidisciplinary environment.

Responsibilitiesinclude medical and surgical trauma response, provision ofpharmacotherapy to adult and pediatric ED patients, preparation ofselect IV products for immediate use, medication reconciliation,and participation in departmental quality improvement projects.Expectation to participate in training of PGY1 and PGY2 pharmacyresidents.

All pharmacists fill orders fordrugs, monitor patient drug therapies and provide drug information.Pharmacists provide pharmacy services with compassion, excellence,and efficiency.

Operational Duties andResponsibilities
Prepares and dispenses drug ordersper physician request according to established policies,procedures, and protocols.
Interprets drug orders(verbal and written) and transcribes/verifies into computerizedpatient medication record accurately. Maintains, accurate, completepatient drug record.
Compounds and dispensespharmaceuticals including non-sterile and sterile products,chemotherapy and parenteral nutrition products accurately.
Issues controlled substances to patient care areas andmaintains records as required by law and institutional policies.
Provides services efficiently and in a timelyfashion.
Maintains current pharmacist license.Ensures applicable CE records and licensure are maintained indepartment files. Attends staff meetings.
Completesall competency/skills assessment requirements.
Supervises and directs pharmacy support personnel. Verifies thedaily activities of pharmacy technicians
Ambulatorycare Pharmacist: Provides ambulatory pharmaceutical care servicesutilizing approved protocols and/or collaborative agreement withother providers, including managing patient drug therapy,anticoagulation management, providing patient education, andpreparing and/or overseeing the preparation and dispensing ofmedications. Performs duties in the clinical areas assigned such asbut not limited to anticoagulation clinics, intensive medical home,transition of care, Discharge Medication reconciliation, etc.

Clinical Duties and Responsibilities
Ensures safe, appropriate, cost-effective drug therapies forpatients according to established policies, procedures, andprotocols.
Monitors drug therapy regimens forcontraindications, drug-drug interactions, drug-food interactions,allergies and appropriateness of drug and dose.
Assists with pharmacokinetics consult service and renal drug dosingper hospital protocol.
Reads, extracts, andinterprets information in patient medical records accurately.
Detects and reports suspected adverse drug reactionsaccurately and in a timely manner.
Sustains thehospital drug formulary, minimizing non-formulary procurements,utilizing therapeutic substitution protocols, and promotingrational drug therapy selection.
Provides clinicalconsultation and clarification to practitioners asappropriate.
Provides accurate, adequate, and timelydrug information to the hospital's professional staff.
Provides drug education to patients and their families perinstitutional protocol.
Participates in the qualityimprovement and medication use review activities of the department.Collects data, conducts monitors and inspections, and maintainslogs, records and other documentation as assigned.
Conducts designated interventions as defined in department ClinicalIntervention activities (IV to PO, Renal Dosing, Pharmacokineticdosing support, Anticoagulation). Baseline departmental standardsfor reporting interventions for clinical productivity as defined bydepartmental goals.
Participates in antibioticstewardship, opioid stewardship, and shortage management activitiesas assigned.
Attends interdisciplinary rounds whenassigned.
Provides services efficiently and in atimely fashion.

Potential for exposure to hazardous and toxic substances(including chemotherapeutic, cytotoxic drugs and cleaningsolutions), sticks or cuts by needles and other sharp items.

Potential formusculoskeletal injuries if proper lifting and carrying techniquesare not used.
Potential exposure to biologic materialwhen participating in direct patient care activities (e. g.,patient education, code attendance).

Experience:

  • Postgraduate Year One (PGY1) Pharmacy PracticeResidency or Commensurate Board Certification or Commensurateexperience
  • Postgraduate Year Two (PGY2)Specialized Residency in area of specialty -preferred

Requirements:

  • Doctor of Pharmacy or BS Pharmacy degreerequired.
  • Hospital experiencerequired.
  • Current Pharmacist licensewith Texas State Board of Pharmacy(TSBP)

  • Sterile Products (IV)Certification through the Accreditation Council for PharmacyEducation (ACPE) within 60 days ofhire.

  • BLS or ACLS and PALS(pediatric practice) within 180 days of startdate.
